Post-Hospital Care: A Collaborative Service between Equinoxe and the Waldorf

 

Equinoxe Lifecare Solutions and the Waldorf Residence are providing a new program of special Post Hospital Care services for individuals who prefer to recuperate in privacy and comfort, following surgery, treatments and/or hospitalization.

 

Situated in specially appointed private rooms at the Waldorf, Equinoxe provides support in activities of daily living such as personal care and hygiene, walking and gentle exercise, skin care, nutrition and comfort services like massage, all of which contribute to the well being of the client and relieves the physical and emotional stress for the family.


A full range of services make convalescence as easy and comfortable as possible.

 

These services include:


• Care Management
• RN on call at all times
• Three meals a day
• Personal care such as bathing, showering, and grooming
• Transfers, repositioning and operation of mechanical lifts if necessary
• Toileting assistance including incontinence care
• Nutrition counseling and monitoring of special diets
• Personal laundry, light housekeeping
• Skin and Mouth care
• Colostomy and catheter care
• Wound care such as dressing changes
• Regular Monitoring of vital signs including respiration, weight, glucose,
• Data collection on regular basis via dedicated monitoring system with linkages to hospital and doctor if required
• Necessary supplies such as piques, bandages, wheelchairs
• Companionship
• On site attention and service 24 hours a day, 7 days a week


À la Carte services offered through Equinoxe:

 

• Additional RN hours: 1:1 care
• Additional PAB hours: 1:1 care
• Care Management visits
➢ Daily Visits
➢ Daily progress reports to family/relatives (e-mail and/or phone)
➢ Communicate with interdisciplinary team: doctors, therapists, social workers.

• Social Stimulation/Therapy

Feeling overwhelmed, lonely and even a little down?

 

Join our new family caregiver support group and:

     Break free from social isolation,
     Learn simple techniques for improving your mood,
     Share with and learn from like-minded individuals facing similar challenges

 

Groups will be facilitated by a psychotherapist who has beentrained in cognitive behavioural therapy. This method has recently attracted a great deal of attention for its proven effectiveness in helping a wide variety of individuals facing an array of challenges in a relatively short period of time.
